Awareness Counseling Center

• ACC online is available by appointment only.
• ACC is available for adults 18 years and older.
• ACC is available in session slots of 15, 30, 45, and 60 minutes.
• ACC is not intended as a substitution face-to-face session.
• ACC is Confidential, and is password protected.
• Every session is conducted by a Licensed Professional Counselor.
• Thursdays and Fridays are the days ACC counselors have the most availability, however ACC cannot guarantee that a counselor will be available at a specific time.
• Just follow the easy steps outlined and you will be on your way  to your ACC appointment.

TERMS AND CONDITIONS 

1.) I agree with, and understand that ACC is not intended for people wishing to engage in psychotherapy of any type. I understand that ACC should be used for education, guidance, and support services for adults ages 18, and older. It is also not intended for individuals with thoughts of suicide, or other self injurious behaviors.  I understand that ACC does reserve the right to consider whether the sessions need more in-depth psychotherapy.
2.)  If we believe you would be better served in another format,  we will inform you immediately!
3.)  I agree that I am 18 years of age or older.
4.)  I agree and understand that ACC is legally responsible for reporting any suspicions that you may harm yourself or others.
5.)  I agree and understand that ACC is legally responsible for reporting any suspicions of child or elderly abuse.
6.)  I agree and understand that courts may have the power to subpoena ACC records if they believe it to be relevant to a particular case.
7.)  I agree and understand that even though Paypal uses a secure server, ACC will not be responsible for any breaches of confidentiality that occur through their web site. You may review their policies at their web site www.paypal.com
8.)  I agree and understand that this service is provided on an "AS IS" basis without warranty of any kind, expressed or implied. You expressly agree that the use of the service is solely your risk. ACC does not warrant that the service will be uninterrupted or error free, nor does ACC make any warranty as to any results that may be obtained by the use of the service.
9.)  I agree and understand that ACC therapists are licensed professional counselors in the state of

and thus for online counseling, and information sharing are not working under the auspices of the

license. ACC therapists will endeavor to be of assistance for providing education, information sharing, guidance, and counsel only. Not psychotherapy. Please do not substitute this for professional help.
10.) I agree and understand that under no circumstances shall ACC be liable to you or any other person for any indirect, incidental, consequential, or punitive damages arising out of or related to this agreement. Your sole and exclusive
remedy here under shall be for you to discontinue the use of this service.
11.) I agree to indemnify, hold harmless, and defend ACC, its owner, its employees, and agents against any action, cause, claim, damage, debt, demand or liability including reasonable costs, and attorney's fees asserted by any person arising out of relating to:
a.) This agreement; b.) your use of the service including any data or work transmitted or received by you; c.) and any libelous, slanderous, indecent or other statement concerning any person made, or republished by you.
12.)  I agree not to engage in unacceptable behavior in the use of this service, which includes without limitation, use of the service to:
a.) Disseminate or transit unsolicited messages, chain letters or unsolicited commercial e-mail; b.) Disseminate or transit material that, to a reasonable person may be abusive, obscene, pornographic, defamatory, harassing, grossly offensive, vulgar, threatening, or malicious; c.) Disseminate or transit, graphics, software, of other material that actually or potentially infringes the copyright, trademark, patent, trade secret, or other intellectual property rights of any persons; d.) Export or re-export or permit downloading of any message or content in violation of any export or import law, regulation or restriction of the United States, and its agencies or authorities or without all required authorizations, licenses or exemptions; e.) interfere, disrupt or attempt to gain unauthorized access to other accounts on the service or any other computer network; f.) Disseminate or transmit viruses or any other malicious code or program, or; g.) Engage in any activity deemed by ACC to be in conflict with the spirit or intent of this agreement.
13.)  I agree and understand that ACC shall have the right at any time, and without notice, to add or modify terms of this agreement simply by posting such amended here in. Your use of the service after the day such amended terms are delivered shall be deemed to constitute acceptance of such amended items.
14.)  I agree and understand that ACC will do all that it can to keep current on this security issue. However, ACC cannot guarantee your privacy if a third party computer hacker somehow gains access to the information during or after the transmissions. By agreeing to online counseling you agree to keep this risk in mind and you waive all claims or potential claims that may arise from a breach of your privacy expectations by reason of third party intruders.
15.)  I agree to pay the designated fee according to the amount of time of service I choose, as follows; $15.00 for 15 minutes, $30.00 for 30 minutes, $45.00 for 45 minutes, and $60.00 for 60 minutes. I agree to remit payment before I begin any session. Payment will be made using the online payment system "Paypal".

PROCESS FOR OBTAINING ONLINE COUNSELING SERVICES

• Read Terms and Conditions, if you accept, then continue;
• Set up Paypal account by going to www.paypal.com
• Set up Skype account at www.skype.com
• Contact counselor via form below to request an appointment, please specify days and times preferred.  Therapist will try to accomodate, but please note that therapist may offer alternate dates and times.  Do not make payment until you have agreed upon an appointment time with therapist.
• The lastest that payment for session will be accepted is  15 minutes prior to session.  We recommend that payment be sent at least 24 hours prior to session. Therapist will email an acknowledgement of reciept of payment and confirmation of appt date and time.
• It is client's responsibility to be available and online at scheduled time of visit.  Session will end at time previously arranged.  If client would like additional time, they will have to repeat the above steps to ensure that therapist will be available to work with them.
